I agree with all the replies so far, however please also check the permissions on the Oracle account used to connect to the CMS database.
Make sure it has READ WRITE and DELETE permissions. If possible for testing purposes just give it DBA.
I've seen scenarios where the permissions on the Oracle CMS account have been stripped after the initial setup of the CMS database which may cause similar issues.
